Potentially wild angle to consider: it is a recreation of a child’s scribble on their fridge because the child died.
I assume it’s a kid’s drawing recreation, whether they’re alive or not, but either way it still looks objectively terrible.

It looks like there is a scar in there with a similar shape. I’m hoping they are in a better place now, but this tattoo seems like an odd way to heal.
