It really wasn’t the one time. His handlers had been stage managing his public appearances so he wouldn’t wander off, and he’s said a litany of very clearly odd things in public. That one time just happened to occur on live national TV and couldn’t be spun like the others.

So then if those are standards we’re holding for Biden, why not Trump? Isn’t he constantly doing off things in public AND behind closed doors?
